How do I access forms for Employees, Contractors, and Employers?

There is a link to our Forms page in the upper-right hand corner of the Knowledge Base.

It can also be accessed from the Resources menu of our website at masterpayusa.com.

Employees can complete and submit the following forms on-line:

• Direct Deposit Form
• Form W-4
• Form I-9
• State Withholding Forms 

Contractors can complete and submit the following forms on-line:

• Direct Deposit Form
• Form W-9

Employers can complete and submit the following forms on-line:

• New Employee Setup Form
• IRS Application for EIN
